I use:
* VU+ Duo
* Black hole 2.05 rev B
* CAM Mgcamd-1.35a
On the linux box everything seems ok, I modified newcamdlist and uploaded it with Filezilla.
According to me there is something missing in my router settings, do I have to setup any portforwarding?

You have to disable firewall for your VU+ receiver, otherwise- how would you exchange data with the server? Your receiver must send requests to the server, server will send you the keys to encode the channels you watch.

my vu+ receiver is behind a cisco router with integrated firewall.
the VU+ has internal IP 192.xxx.xx.10
According to the settings in "my subscription" I will have outbound traffic to your servers contacting the ports in the CWS lines.
does this mean I have to open up the same ports in my firewall to let traffic back in?
e.g. outbound traffic to www.serverexample.com port 5434
I should allow a port forwarding for any incomming traffic directed to 5434 towards 192.xxx.xx.10?
I am still confused!
I can also put the vu+ in the DMZ, but then it is accessible and not secure.

If they match to the ports of your router - YES, you have to open them. As I think, port of your router and server port - not the same thing. However, you need to open all incoming and outgoing traffic related to our server.
