Have you tried ALL versions of CSC? You work with 4.0.0.2, you told about 4.0.0.3 and 4.0.0.4, what about 4.0.0.1?
Yes, we changed UPC card because provider changed CAID and replaced all cards available, so what whall we do now? Return the old card back?
Find solution on any UPC forums, ProgDVB forums if you cannot find the solution here.
Finally, reinstall ProgDVB+plug-in and try again.
